And more importantly, there's a very good chance the problem isn't a mystery at all. It's soft tissue — muscles, tendons, ligaments, fascia, and nerves that have become restricted, adhered, or entrapped over time.
That's exactly where Active Release Therapy comes in. I want to take this issue of Back in Motion to walk you through what ART actually is, what it can help with, and what you can expect if you've never experienced a session before. No intimidating language, no vague promises — just a straightforward explanation of one of the most effective hands-on techniques I use here at Mansfield Spinal Care.
So What Is Active Release Therapy?
Active Release Therapy — commonly called ART — is a highly specific, movement-based soft-tissue technique. It was developed by Dr. P. Michael Leahy, a chiropractor who noticed that many of his patients' pain and movement problems weren't coming from the spine alone, but from changes in the quality and texture of the surrounding soft tissues.
Here's the core idea: every muscle, tendon, ligament, sheet of fascia, and nerve in your body has a specific texture and the ability to glide freely against neighboring structures. When tissue gets overworked, injured, or simply stressed repeatedly over time, the body lays down scar tissue and adhesions as part of its natural healing response. The problem is that this scar tissue is denser and less flexible than healthy tissue. It can bind muscles together, restrict nerve movement, limit your range of motion, and create that chronic ache or tightness that just never fully resolves.
ART addresses this directly. During a session, I apply precisely directed tension to the affected tissue with my hands — and then guide you through a very specific movement as I hold that contact. The combination of that controlled tension and the movement of the tissue underneath essentially breaks up the adhesion, restores the tissue's ability to glide, and allows the affected structure to function the way it's meant to.
Every protocol is different depending on which tissue is involved and what's causing the restriction. ART has over 500 distinct treatment protocols covering muscles, tendons, ligaments, fascia, and nerves throughout the entire body. That specificity is what separates it from a general massage or broad manipulation.
What Can ART Help With?
ART is especially well-suited for soft-tissue problems that have either been building up over a long period of time or that developed suddenly from an injury. Here are some of the conditions many patients seek it out for:
Overuse and Repetitive Stress Injuries
If your pain comes on gradually and seems tied to something you do repeatedly — typing, lifting, throwing, swinging a golf club, sitting at a desk for hours — overuse is often the culprit. The soft tissues don't get a true chance to recover between sessions of demand, so micro-damage accumulates and adhesions form. Common examples include rotator cuff tightness, tennis elbow, IT band issues, plantar fasciitis, and carpal tunnel-type symptoms in the wrist and forearm.
Scar Tissue from Old Injuries
Even an injury that "healed" years ago may have left behind dense scar tissue that quietly limits movement or puts ongoing stress on nearby structures. Many patients are surprised to learn that an old ankle sprain or a shoulder strain from a decade ago could be contributing to their current discomfort.
Post-Accident Soft-Tissue Injuries
Motor vehicle accidents are a very common reason people end up in my office, and for good reason. A collision — even one that seems minor — can jar soft tissues in ways that don't always show up on imaging but absolutely affect how you feel and move. Whiplash is the classic example: the muscles, ligaments, and fascia of the neck and upper back absorb tremendous force in a fraction of a second. ART can be a valuable part of recovery from those kinds of soft-tissue injuries.
Headaches
A significant portion of chronic headaches — especially tension-type headaches and those that start at the base of the skull — have a soft-tissue component. Tight muscles and restricted fascia in the neck and upper back can refer pain into the head, compress nerves, and reduce blood flow. Many patients who struggle with recurring headaches find meaningful relief when the underlying tissue tension is addressed directly.
Sciatica and Nerve Entrapment
Sciatica — that radiating pain, numbness, or tingling that travels from the lower back or hip down into the leg — often involves a nerve that's being compressed or restricted somewhere along its path. Sometimes that's a disc issue, but in many cases, it's the surrounding soft tissue that's the problem. The sciatic nerve has to glide through and alongside multiple muscles on its way down the leg. If any of those muscles are tight or adhered, the nerve can't move freely, and symptoms follow. ART includes specific nerve-mobilization protocols that address exactly this kind of entrapment.
What Does a Session Actually Feel Like?
This is the question I get most often from patients who are curious about ART but have never tried it. Here's an honest answer: it's not like a relaxing massage, but it's also not as intense as some people imagine.
I'll start by feeling the tissue — its texture, tightness, and how it moves relative to the structures around it. When I locate an area of restriction or adhesion, I'll apply a firm, specific contact with my thumb or fingers and ask you to move the affected area — for example, slowly straightening your arm, turning your head, or extending your leg — while I hold that contact. As the tissue moves under my hand, you'll often feel a distinct sensation in that spot. Some people describe it as a "good hurt" — a strong but tolerable feeling that they recognize as the right area. Some spots are more sensitive than others depending on how much restriction is present.
Sessions are typically focused and efficient. We're not working the whole body in one visit — we're targeting the specific structures involved in your complaint. After treatment, many patients notice an immediate change in how the tissue feels or how much range of motion they have. That's not always the case, and I'd never promise a specific result, but improved mobility right off the table is something many patients experience after their first few sessions.
There can be mild soreness in the treated area for a day or two afterward — similar to what you might feel after a tough workout — and that's completely normal. It's a sign the tissue is responding.

It's broken into separate courses covering the upper extremity (shoulder, elbow, wrist, hand), lower extremity (hip, knee, ankle, foot), spine, and nerves. This Week's Actionable Tip: The Nerve Glide for Sciatic Relief
If you deal with sciatica or radiating leg pain, nerve mobility exercises — sometimes called nerve glides or neural flossing — can help keep the sciatic nerve moving freely through the surrounding soft tissue. Here's a simple version to try:
Sit upright in a firm chair with both feet flat on the floor. Slowly straighten one knee until your leg is extended out in front of you, then gently flex your foot (toes toward you) and hold for two seconds. Lower your leg back down and repeat on the other side. Do five to eight slow, controlled repetitions per side once or twice a day.
The goal is gentle movement — not a big stretch, and definitely not pushing through sharp or worsening pain. If straightening your leg brings on strong symptoms, stop and let us take a look before continuing. When done correctly, many people feel a mild pulling sensation along the back of the leg that eases with repeated motion. That's the nerve beginning to glide more freely. It's a simple habit that complements soft-tissue care beautifully and costs you nothing to try.
